Getting Started with California Conceptions

 

We understand that waiting for donated embryos can be a long and frustrating process. Our program is designed to speed up the normally lengthy process of waiting for donated embryos to become available. Instead of waiting years, most recipients can expect to receive their first anonymous donor embryo match within 2-6 months. Getting started is simple.

Steps to Starting the Process

The California Conceptions Donor Embryo Program is operated by California IVF Fertility Center.  Patients will need establish care with our center before proceeding forward.  Contact our clinic to set up a discussion with one of the California Conceptions coordinators.  The coordinator will review the program in detail.  Patients that are still interested in going forward with the donor embryo program will be scheduled for a consultation with one of our physicians.  After the new patient consultation, patients will complete their testing and clinical psychology consultation.  When the pre-cycle testing and counseling is complete, the matching process can begin.

Initial Contact

The initial step in starting the process for California Conceptions Donor Embryo Program is to make contact with one of our patient coordinators.  The patient coordinator will review the details of the California Conceptions Program.  During this review, patients will learn about the program, steps needed to coordinate care, and the matching process.  Additional details will include a review of the guarantee eligibility as well as an overview of the treatment process.  Genetic screening of embryos is also discussed as an option.  Patients should use this opportunity to ask any questions about the California Conceptions Program before making the final decision to proceed.

Physician Consultation

After reviewing the details of the California Conceptions Donor Embryo Program, patients deciding to proceed forward with treatment will be scheduled for a consultation with one of our physicians.  During the consultation, the physician will review your health history and treatment history.  The physician will briefly review the medical aspects of the treatment with donor embryos, and can also review alternatives such as donor eggs. and embryo adoption.  The physician may request additional testing before approving patients to move forward with treatment.  After the consultation, the treatment coordinator will follow up with the patient to make sure all testing and requirements are met before the matching process.

Testing & Results

There are routine tests that we require of all patients.  The California Conceptions physician may require additional testing or consultations before patients can proceed with the donor embryo program.  It is important to complete all of the necessary testing and any additional requirements so we can minimize any risks to the mother and baby during pregnancy.  Test results and copies of any requested consultations will be required before the matching process can begin.  They California Conceptions physician does not generally need a complete set of medical records or treatment records unless specifically requested by the physician.  Patients with certain medical conditions or complications may need to provide additional records for review.

Counseling

The California Conceptions Donor Embryo Program follows the American Society For Reproductive Medicine’s (ASRM) guidelines for third party reproduction for donors and recipients.  Recipients of donor embryos are required to undergo counseling with a mental health provider familiar with, and experienced with, ASRM’s recommendations for counseling.  This is not the same as marital counseling or counseling for infertility. Our clinic must receive a copy of the provider’s letter or notes documenting the counseling.

Matching

Once a patient has completed their consultations, testing, and counseling, the matching process can begin.  The match may take several months to occur, though in most cases the wait time is relatively minimal.  The quicker patients complete their required testing and counseling with the mental health provider, the sooner we can proceed with a match.

Matching is done by taking into consideration the physical characteristics of the parents, including ethnicity, photographs, and written preferences on the profile form.  Patients with very strict criteria for donor selection should consider egg donation as an alternative to donated embryos.

Getting to the Match Faster

 

If you would like to speed up the process of getting to your donor embryo match, there are several things you can do in advance.  The steps outlined below will help your initial consultation and evaluation go much quicker.  Completing the necessary steps will allow us to present you with a possible matches with a donor embryo.

 

Complete Your Application

The first step is to complete your online application for the donor embryo program.  To speed up the process, please complete the online patient history form as well.

When you are completing your online health history, please provide complete details.  Be sure to list any medical conditions, medications, surgeries, and provide a nice summary of previous fertility treatments.  An example can be seen below.

We typically do not need complete embryology records when patients provide us with a good summary.  Since we are using donor sperm and eggs, we are more concerned with records or a patient summary describing any problems with the uterus.

Example summary:

Testing:  Normal hysteroscopy 3-2018.  Endometrium developed normally during IVF cycles.  Had problems with getting lining to grow during frozen cycle.

Treatments:
2 IUI cycles in 2017 using husbands sperm.
IVF 5-2018 with 8 eggs retrieved, 6 fertilized, 2 day 5 embryos transferred, 1 frozen embryo.  Doctors said I had poor egg quality.  Negative test.
Frozen embryo cycle 7-2018 with 1 embryo, live birth.

Medical Consultations

To assist our physicians with your health history, you can have your physician perform a complete history and physical.  Forward the documentation of this history and physical.  Preferably, the physician will be the same one that will provide obstetrical care for you in the event of a pregnancy.  Ask your physician to provide a statement that you have been cleared for pregnancy and they will provide care for you in the future.  If any additional physician consultations are required to clear you for pregnancy, please complete these steps and provide us with the documentation.  This helps reduce the time that may be required for extra testing to confirm that you are healthy and ready for a safe pregnancy.

We typically do not need complete copies of your medical records or fertility treatment records other than information pertaining to recent blood tests or tests looking at the uterus.  If you have had a consultation with a specialist for health reasons that could affect your pregnancy, we would like to receive copies of the consultations and a physician statement clearing you for pregnancy.

Testing and Results

There are routine tests that are recommended prior to pregnancy.  You can get these tests in advance if you would like to shorten the wait time for donor embryos.  Please keep in mind that our physician may order additional lab testing depending on your health history.  We will provide you a list of the required labs for our program after your consultation with one of our physicians.  We can also use any labs that you already have as long as they are current within the last year.

You can also assist our physicians by updating any required screening tests such as PAP smears and mammograms (if indicated for your age).  Having these records available when the physician reviews your history can be a great help.

Patients will also need to complete a saline hydrosonogram, also called a saline contrast ultrasound.  There are some cases when this isn’t necessary.  Patients wanting to qualify for the pregnancy guarantee using donor embryos will need to have their saline contrast ultrasound performed at our clinic.  For patients not trying to qualify for the refund option, the saline contrast ultrasound and physical exam form can be used at your own clinic.

Counseling

Before using donor embryos or donor eggs, patients will need to complete a counseling session with a mental health provider experienced with the American Society For Reproductive Medicine’s (ASRM) guidelines for third party reproduction for recipients.  The goal of this counseling is to discuss topics relevant to using donor eggs or donor embryos.  ASRM provides guidance to mental health providers regarding topics that should be covered during this discussion.  Both partners (if applicable) will need to participate in this counseling and we will need to receive a letter from the mental health provider that the relevant topics have been covered.

You can complete the counseling in advance.  If you have already completed this step, please forward the documents for our review. If the documentation is adequate, patients will not have to repeat the counseling.  Your mental health provider should have experience with ASRM guidelines, otherwise the counseling may not satisfy this requirement.  Open an outline of the counseling for recipients of donor embryos here.
